Bath and North East Somerset Child and Family Health

Launched in April 2025, the Bath and North East Somerset (B&NES) public health nursing contract builds on HCRG Care Group’s long-standing role as system leader in the area. Now with a single point of access, and a digital front door, a newly adopted model is creating accessible, high quality care for families. We’re also changing community health for good through a care leaver reverse mentoring scheme and digital questionnaires that enable us to offer more targeted school nurse support.

BSW Community Health

Launched in 2025, the Bath & North East Somerset, Swindon and Wiltshire (BSW) contract covers every stage of life: from pre-birth to end-of-life. Acting as system leader, HCRG Care Group is working with the third sector and other partners to shift care from hospital to community, analogue to digital, and treatment to prevention. We’re creating a new template for changing community health for good: with a single point of access, workforce transformation, and success based on outcomes.

Cheshire West and Chester Sexual Health

Since 2019, HCRG Care Group has delivered sexual health services across Cheshire West and Chester, serving a population of 357,000. Rated Good overall and Outstanding for Responsiveness by the CQC, the service prioritises prevention through targeted outreach and visible high street clinics with integrated digital services through SH:24. By training GPs, pharmacies and VCSE partners and tailoring care to local needs, we are reducing health inequalities and helping people take more control of their health.

Coventry and Warwickshire Sexual Health

In 2024, HCRG Care Group launched the Heart of England Sexual Health (HESH) service for Coventry and Warwickshire, serving almost one million people. With £2m invested in new high street clinics, and a fully integrated digital offer via SH:24, the service is raising visibility and delivering discreet, high-quality care. Digital uptake is strong, and we’re expanding outreach capacity to build prevention further into our services and the wider community.

Essex Child and Family Wellbeing

Since piloting outcomes-based commissioning with HCRG Care Group in 2018, the Essex Child and Family Wellbeing contract is now fully aligned with the national Common Outcomes Framework. Through Family Hubs and partnerships, Essex is showing how integrated services beyond a building can join up health, housing, education and social care to deliver high standards of care – and change community health for good.

Lancashire Child and Family

Since 2019, HCRG Care Group has delivered the Healthy Child Programme across Lancashire, bringing health visitors, school nurses and safeguarding specialists together with local partners through Family Hubs. We are expanding mandated health reviews, investing upfront in the quality of the workforce and launching a new digital front door – codesigned with young people who are increasingly shaping our services directly.

West Lancashire Community Services (Adult)

Since 2017, HCRG Care Group has delivered adult community services across West Lancashire, focusing on prevention, reducing avoidable admissions and bringing care closer to home. We’ve led the development of integrated neighbourhood teams (INTs) that have strengthened partnerships between care homes, district nurses and wider partners – enabling early intervention and continuous improvement, and helping the NHS in West Lancashire to fulfil its aspirations for a high quality, community-based care system.

Leicestershire & Rutland Sexual Health

Since 2025, HCRG Care Group has been deliver a new integrated sexual health service for Leicester and Rutland. The focus is on investing in prevention through a revitalised hub in Loughborough, expanded spoke clinics and targeted outreach across the counties. Together with a new single point of access, and a dedicated partnership with UX organisation SH:24, education and services are becoming more visible and accessible and are changing community health for good.

Oldham, Rochdale & Bury Sexual Health

Since 2016, HCRG Care Group has led sexual health services across Oldham, Rochdale and Bury, serving a population of over 650,000. The service combines local collaboration with national-benchmarks and data resources to provide a fully integrated digital model and targeted clinical outreach. Rated Outstanding for Caring and Good overall by the CQC, we’re now continuing to improve quality and build resilience across the system.

Slough Child and Family Wellbeing

Since 2025, HCRG Care Group has led the Healthy Child Programme in Slough, integrating 0–19 services with Early Help through Family Hubs. In one of the UK’s most diverse and youngest populations, we’re co-designing with local partners to deliver joined-up care and outreach. We’re also pioneering digital access and creating new pathways such as the area’s first fully integrated antenatal programme – changing community health for good.

Surrey Child and Family Wellbeing Service

In 2025, HCRG Care Group began to deliver an integrated children’s service in Surrey. We are addressing waiting times and building digitally enabled neighbourhood teams that support children and families across the county. The focus is on transformation that makes a new standard of service possible: so families only need to tell their story once, access support more easily, and receive consistent high quality care no matter where they live.

Wiltshire Children and Families Wellbeing

HCRG Care Group delivers Wiltshire’s Public Health Nursing service – health visiting, school nursing, baby clinics and the Family Nurse Partnership – alongside a full range of specialist children’s services. Community health is changing for good through a new digital front door that offers families a direct messaging service to health visitors, the introduction of equality and engagement leads, and improved immunisation uptake through digital consent.
