Creating better lifelong health outcomes by achieving three radical shifts in adult health

Local authorities and the NHS are increasingly measuring success by outcomes rather than outputs, and for a long time this is how we’ve looked at designing community services. Are adults living independently for longer? Are health inequalities narrowing? Are people confident and supported in managing their health?

Together, we create community-first services that mean adults can stay healthier for longer, with seamless access to the right level of care at the right time.

How we’re co-designing the future of care

By investing in a digital front door and neighbourhood teams, working with VCFSE partners, and setting up multi-disciplinary triage with a single point of access, we can ensure people are fast-tracked to urgent care when needed but are otherwise supported in the community rather than in a hospital.

This opens the way for smarter care pathways that prioritise prevention. Combined with third sector partnerships and estate reform – we can deliver services at 10% lower cost without compromising quality or consistency. In BSW, for example, we transferred over 2,000 staff and 100+ service lines from nine providers without disruption.

The systems at work

Designed according to need

Being commissioned against an outcomes framework means our teams have the freedom to design services based on the community’s needs, taking into account commissioning priorities, children’s population health data and community engagement.

Pathway redesign

We redesign pathways that integrate fragmented services into needs-based models (such as frailty, foot health, or neurodiversity) to make care more personalised and create room for new efficiencies.

Continuity of care

This ensures people only have to tell their story once, and personalised care plans are followed across multi-disciplinary teams in a true integration of services.

Local people have more control

We make it easier for them to proactively manage their conditions rather than simply joining waitlists, as well as self-refer to access the community services they need.

Neighbourhood teams

We use data and population health tools to target interventions where they matter most – and are appropriately trained in trauma-informed, personalised care approaches, ensuring care plans reflect what matters to people, not just their diagnoses.

Smart delivery model

This enables us to step patients up from community to specialist intervention without defaulting to acute admissions, making the most of community beds and remote monitoring.

System leadership

We actively drive collaboration across existing community providers for system-wide benefits, as well as bringing in new strategic partners such as the Stroke Association to own specialist care pathways.

Community partnerships

We embed small charities and social enterprises into delivery, and codesign with local people and organisations, ensuring our services feel embedded rather than imposed.

Upfront investment

We invest into infrastructure and transformation – such as in BSW where we are investing over £8m to deliver change in two years rather than ten – without requiring extra funding in the initial years of a contract.

Adult services for ICBs

We work with ICBs to join up and streamline adults’ services, ensuring those with additional needs receive personalised care that results in improved outcomes and reduced health inequalities.

We also support other organisations such as foundation trusts to deliver on these contracts to ICBs.
