Health and care achievements celebrated

Successfully launching new services and delivering transformation projects to improve the lives of patients and service users are among the achievements celebrated in HCRG Care Group’s Quality Accounts for 2023/24.

The Quality Accounts detail the quality of services provided by HCRG Care Group and inform our stakeholders about the standard of care that can be expected from services, how it measures the quality of services and plans for further improving the quality of care provided.

HCRG Care Group is one of the UK’s largest independent community services providers and has a strong track record of changing people’s lives by transforming health and care. It runs services across the country on behalf of NHS and local government commissioners, including adult and children’s community services, sexual health, offender and custodial health, primary and urgent care and integrated 0-19s services.

Amongst the achievements in 2023/24 were:

  • Developing Integrated Neighbourhood Teams in West Lancashire
  • Launching a Community Transfer of Care Hub to support patients coming out of hospital in North Kent
  • Supporting the wider health and care system in Bath and North East Somerset to support urgent care and flow including the implementation of virtual wards
  • Successfully launching two new contracts in Essex – West Essex Special Educational Needs and Disabilities Therapies in May 2023 and North East Essex Community Paediatric Service in September 2023
  • Working collaboratively with partners In Wiltshire to proactively manage and support children, young people and families with increasing levels of health and wellbeing concerns
  • Introducing weekly pressure ulcer huddles in Surrey to support patients at risk of or suffering pressure ulcers
  • Achieving gold standard in UNICEF’s Baby Friendly Initiative in Lancashire for the fourth year running, in partnership with Lancashire County Council and Families and Babies (FAB)
